For World Sight Day 2020, COS is inviting Canadians to find out their risk for developing one of the four serious eye diseases by taking a quick and simple assessment on the COS website at www.seethepossibilities.ca. By taking the test, participants are automatically entered to win one of fifty gift certificates of $250 each from BonLook, a Montreal-based eyewear brand specializing in trendy, affordable prescription eyewear.

OFFICIAL CONTEST RULES AND REGULATIONS
The Know the Risks Contest (the “Contest”) is brought to you by the Canadian Ophthalmological Society (“COS”) and BonLook (collectively the “Sponsors”). COS is the advocate for vision health for all Canadians, an affiliate of the Canadian Medical Association and an accredited provider of Continuing Professional Development, as recognized by the Royal College of Physicians and Surgeons of Canada. BonLook is a leading optical brand with more than 30 stores as well as an online boutique. By entering the Contest, you acknowledge that you have read, understood and agree to the terms and conditions below. No purchase is necessary to enter or win. Contest begins at 12:01 AM E.D.T. on Friday, August 21, 2020 and ends at 11:59 PM E.D.T. on Sunday, October 18, 2020 (“Contest Period”), after which time the Contest will be closed and no further entries will be accepted.
1. Eligibility: Contest is open to all Canadian residents having reached the legal age of majority in their province or territory of residence prior to the Contest Period (an “Eligible Participant”). Employees, representatives or agents of the Sponsors, and each of the foregoing entity’s subsidiaries and affiliates, media partners, advertising and promotional agencies, and members of the immediate family (parents, children, siblings and spouse, regardless of where they are domiciled) of any of the aforementioned and persons with whom any of the aforementioned are domiciled are not eligible to enter the Contest. By participating in this Contest, Eligible Participants acknowledge compliance with, and agree to be bound by, these Contest rules. Eligible Participants who do not comply with any of the Contest rules are subject to disqualification by the Sponsors, in Sponsors’ sole discretion.
2. How to Enter: NO PURCHASE NECESSARY. Online access is required in order to participate. You can enter by taking the assessment test on www.seethepossibilties.ca and filling out your personal information on the entry ballot at the end of the test. Each test taken and ballot filled by an Eligible Participant during the Contest Period will count as one Contest entry (an “Eligible Entry”). Limit one Eligible Entry per Eligible Participant. For this Contest receipt of an Internet entry occurs when Sponsor’s server records the entry information. All entries must be complete and are subject to verification by the Sponsor, in its sole discretion. Duplicate entries, or near duplicate entries, will result in disqualification from the Contest, at the Sponsors’ sole discretion.
Eligible Participants must participate using a single email address. Any attempt or suspected attempt to be use multiple accounts will be considered ineligible and disqualified from the Contest. Automated and/or repetitive electronic submissions (including but not limited to entries made using any script, macro, bot, contest service or otherwise illicit means) will be automatically disqualified and transmissions from these may be blocked by Sponsors. Eligible Entries that contain false or incomplete information are void. Eligible Entries that are late, lost, stolen, illegible, contain false information, are damaged, misdirected, mutilated, garbled or incomplete, altered or otherwise irregular or that do not conform with or satisfy any or all of the conditions of the Contest rules are void.
This Contest is in no way sponsored, endorsed or administered by, or associated with Facebook Inc. or Instagram, LLC. Eligible Participants understand that all Contest information is provided to the Sponsors, and grant Sponsors a non-exclusive licence to use all Eligible Entries for any purpose.
The Sponsors reserves the right, in their sole discretion, to disqualify any individual who is found: (i) to be tampering with the entry process or the operation of the Contest; (ii) to be acting in violation of the Contest rules; (iii) to be acting in an un-sportsmanlike or disruptive manner; (iv) to be in violation of any of the Contest rules specified herein; or (iv) to be engaging in any other offensive conduct that jeopardizes the integrity and/or administration of the Contest, which shall be determined at the Sponsors’ sole discretion.
If the identity of an entrant is disputed, the authorized account holder of the e-mail address submitted at the time of entry will be deemed to be the Eligible Participant. The individual assigned to the e-mail address for the domain associated with the submitted e-mail address is considered the authorized account holder. A selected entrant may be required to provide proof that they are the authorized account holder of the e-mail address associated with the selected entry. The sole determinant of time for the purposes of receipt of a valid entry in this Contest will be the Contest server machine(s).
No correspondence will be undertaken except with selected Eligible Participants whose Eligible Entries were drawn. Once an Eligible Participant is declared a winner by the Sponsors, the Sponsors will send the Contest Winners a letter containing the details on how to redeem their Prize.
3. Prizes: There are a total of fifty (50) prizes to be won (each, a “Prize” and collectively, the “Prizes”) all consisting of gift-certificates to BonLook and BonLook.com. Prizes will each have a retail value of $250 (CAD), for a total amount of $12,500 (CAD) in gift certificates to be won. There is a limit of one (1) Prize per household. COS and BONLOOK reserve the right to substitute a prize of equal or greater value. Contest winner must be at least eighteen years of age, or the age of majority in the province or territory of residence, at the time of entry into the Contest.
The prizes are non-transferable and/or not redeemable for cash or services and cannot be combined with any other special offer or promotion. In the event that a Contest Winner is unable to redeem any or all of the Prize, no compensation or substitute prize will be provided. Redemption of gift-certificates will be subject available inventory and redemption must be before expiration date. No extensions will be granted beyond the expiration date. The Sponsors are not responsible for additional shipping fees, and/or any other incremental fees incurred by Contest winners in the redemption of the Prizes. The Sponsors are not responsible for lost or stolen prize certificates.
All winners assume any and all liability for any injury or damage caused, or claimed to be caused, by entering, participating in this Contest or the use or redemption of a prize. Prizes must be accepted as awarded and have no cash value. Prizes or any portion of a prize cannot be combined with other discounts, promotions or special offers. Sponsor reserves the right to substitute an alternate prize of equal or greater value if an advertised prize is unavailable at the time of award.
4. Odds of Winning: The odds of winning depend on the number of Eligible Entries received before the Contest end date (October 18, 2020 at 11:59 PM E.D.T).
5. Prize Drawing: On October 19, 2020 at 9:00 AM E.D.T. at COS’ Agency’s Offices in Toronto, Ontario (“Draw Date”) the Sponsors, or an employee, agent or other representative of the Sponsors, will conduct a random draw from all Eligible Entries received before the Contest end date and select the fifty (50) potential winners (the “Contest Winners”). The Sponsors or an employee, agent or other representative of the Sponsors, will attempt to contact the Contest Winners via the email address provided on the entry ballot within 3 business days following the draw. Contest Winners must confirm acceptance of the Prize by replying to the email within two (2) business days of transmission by Sponsors of the email. If the selected Eligible Participant whose Eligible Entry was drawn does not or cannot accept a Prize within that time, the selected entrant will be deemed to have forfeited their Prize, and Sponsors shall choose another entrant from the remaining Eligible Entries, by random draw. This process shall continue until each Prize has been successfully awarded. Sponsors shall not be responsible for the failure for any reason whatsoever of a Contest Winner to receive notification or for the failure of Sponsors to receive a Contest Winner’s response.
6. Skill-testing Question: Before being declared a Contest Winner, the Eligible Participant will be required, as a condition of winning a Prize, to correctly answer, without assistance of any kind, the Sponsors’ time-limited, mathematical skill-testing question.
7. Release and Indemnification: Contest Winners must sign the Sponsors’ release (the “Winner Release”) to:
(i) confirm compliance with all Contest Rules and eligibility requirements set out herein;
(ii) agree to accept Prizes as awarded;
(iii) acknowledge the Sponsors’ right to publish their picture, name and city of residence
without compensation other than the Prize offered;
(iv) release, discharge and hold harmless the Sponsor, its departments and agencies, parent, related and affiliated companies, subsidiaries, franchisees, advertising and promotional agencies, counsel, marketing partners, and each of their respective directors, officers, employees, shareholders, successors, sponsors, partners, licensees, subsidiaries, agents, artists, advisors, assignees, and all others associated with the administration, development and execution of the Contest (the “Released Parties”) from and against any and all manner of action, cause of action, claim or demand, loss or injury, use or misuse of a prize or any travel related thereto, and the use of entries by the Sponsor, suit, debt, covenant, contract, including legal fees and expenses, whatsoever, including but not limited to, claims based on negligence, breach of contract and fundamental breach, failure of any third-party contractor or supplier used in connection with any aspect of the Contest to perform or deliver any goods or services, any act of God or any other event beyond the Released Parties’ control, any dissatisfaction of any kind by a winner with any aspect of the Contest or any prize, liability for physical injury, death, or property damage which the entrants, guests, their heirs, successors or assigns have, might have or could have suffered, by reason of or arising out of the entrant’s participation in the Contest and/or in connection with the acceptance and/or exercise by the entrant of prizes as awarded; and
(v) indemnify the Released Parties against any loss, damage or expense, including legal fees, that any of the Released Parties may suffer or incur as a result of any non-compliance by entrants with any of the Contest rules or participation in the Contest and/or in connection with the acceptance and/or exercise by an entrant of a prize, and the use of entries by the Sponsors.
Sponsors are not responsible for:
(i) incorrect or inaccurate entry information that may affect a person’s ability to participate in the Contest or be awarded a Prize, including but not limited to human error, technical malfunctions, lost or delayed entries for any reason, mail failures, omission, or any combination thereof, and entries that fail to fully comply with these Contest Rules;
(ii) technical failures of any kind, including, but not limited to malfunctions, interruptions, or disconnections in phone lines or network hardware or software;
(iii) lost, incomplete, delayed, mutilated or misdirected entries or Winner Releases;
(iv) injury or damage to entrants’ computers or to any other individual’s computer related
to or resulting from participating in, or downloading any material regarding the Contest or accepting a Prize;
(v) any injury or damage to persons or property which may be caused, directly or indirectly, in whole or in part, from entrant’s participation in the Contest or receipt or use or misuse of any Prize, including any related travel and the use of entries by Sponsor;
(vi) the security or privacy of information transmitted via computer networks or for breaches of privacy due to interference by third-party computer hackers or otherwise; or
(vii) late, lost, misdirected or unsuccessful efforts to notify a potential winner.
Sponsors further assume no responsibility for computer system, hardware, software or program malfunctions or other errors/failures of any kind, delayed computer transmissions, network connections, or damages to participants’ computers arising out of participation in this Contest. Sponsors are not responsible for any technical or human errors that may occur in the processing of the entries. Sponsors reserves the right in its sole discretion to cancel or suspend this Contest should virus, bug or other causes beyond Sponsors’ control corrupt the administration, security or proper play of the Contest.
8. Participation Conditions: By participating in this Contest, or accepting the Prize, each entrant releases and holds harmless the Sponsors and their respective subsidiaries, affiliates, and each of their respective parent companies and each such company’s officers, directors, employees, representatives and agencies (collectively, “Sponsors Representatives”) from any and legal claims, demands, damages, action and/or causes of action, all liability or any injuries, loss or damage of any kind arising from or relating to participation in this Contest or acceptance, use, misuse or nonuse of the Prize or parts thereof, and from liability to any other persons relating to or resulting from entering this Contest (collectively the “Claims”). By participating in this Contest, the Contest Winner and all entrants grant to the Sponsors the right, but not the obligation, to use their name, email address, postal code, social media handle(s), comments and image (whether on videotape, photograph or any other means), likeness, and/or voice recording for advertising and promotional purposes, in all media, without notice, additional compensation or permission, except where prohibited by law, and release the Sponsors, and all Sponsors representatives, from and all liability thereto.
By entering this Contest, all entrants accept and agree to be bound by these Contest rules and the decisions of the Sponsors which are final, binding and conclusive on all matters relating to this Contest. All entries become the property of the Sponsor. Sponsors reserve the right to disqualify any entrants who fail to follow these official rules and regulations or who make any misrepresentation relating to the Contest, drawing and redemption of the Prize. This Contest is subject to all applicable federal, provincial and municipal legislation.
In the event of any discrepancy or inconsistency between these Contest rules and any other statements contained in any Contest-related materials, the terms and conditions of these rules shall prevail, govern and control to the fullest extent permitted by law.

9. Governing Law: Contest is void where prohibited by law and is subject to all applicable Canadian federal, provincial, territorial, municipal and local laws. For residents of Quebec, any litigation respecting the conduct or organization of a publicity contest may be submitted to the Régie des alcools, des courses et des jeux (the “RACJ”) for a ruling. Any litigation respecting the awarding of a prize may be submitted to the RACJ only for the purpose of helping the parties reach a settlement. For residents of all other jurisdictions, this Contest shall be governed exclusively by the laws of the Province of Ontario and the federal laws of Canada as applicable therein, including all issues and questions concerning the construction, validity, interpretation and enforceability of these Contest rules, rights and obligations between entrants and the Sponsor, and procedural provisions, without giving effect to any choice of law or conflict of law rules.
10. Rule Amendments: The Sponsors reserve the right, in their sole discretion, to amend or modify these Contest rules, or modify, cancel or suspend this Contest, without prior notice for any reason whatsoever, including without limitation in the event that any cause beyond the reasonable control of the Sponsors corrupts, or threatens to corrupt, the security or proper administration of the Contest. Sponsors further reserve the right to withdraw the transmission of this Contest at any time and award prizes based upon entries received prior to date of the withdrawal.
11. Privacy: Personal information is being collected by Sponsors from entrants and will be used by Sponsors for the purpose of administering this Contest and in accordance with Sponsors’ privacy policies, which can be accessed at https://www.seethepossibilities.ca/privacy-policy. Personal information may be collected, used or disclosed in jurisdictions other than Canada. Personal information that resides in jurisdictions other than Canada will be subject to the general laws of application in those jurisdictions. Sponsors will not sell, share or otherwise disclose personal information of entrants with third parties or agents, other than to third parties or agents engaged by Sponsors to fulfill the Contest or as permitted or required by the applicable laws.
If the entrant checks the applicable box on the entry form for this Contest, the entrant opts in to receive communications, including emails, from Sponsors about it and its products, services, events, news, updates, offers, promotions, customized ads and more. The entrant may withdraw consent at any time by sending an email to communications@cos-sco.ca with the subject line “Consent Withdrawal” from the email address that is associated with your entry.
12. Intellectual Property: All intellectual property, including but not limited to trademarks, logos, designs, promotional materials, web pages, source codes, images, drawings, illustrations, slogans and representations are owned by the Sponsor. All rights reserved. Unauthorized copying or use of any of the Sponsor’s intellectual property without the express written consent of the Sponsor is strictly prohibited.18
